Actual answer: Genetic factors, gut microbiome, sleep patterns, stress responses, food environment, social norms, economic constraints, metabolic adaptation, hormonal regulation, inflammation, dozens of nutrient interactions, environmental toxins, and more.
The simple answers lead to simple interventions that fail for most people. Then we blame the people for not following simple advice, rather than questioning whether the advice was too simple.
The Difference Between Simple and Simplified
Simple: The phenomenon itself has few moving parts. A pendulum is simple. Its behavior follows from a handful of variables.
Simplified: The phenomenon is complex, but we've reduced our description of it. The economy is complex. "Supply and demand" is a simplification.
